Commander's Award for Public Service for FRG Leader

We are a reserve unit on our last stretch of our current deployment, our FRG Leader has been spectacular and my Commander wants me to get all the information I can about an award for her. I'm looking at the "Commander's Award for Public Service". She easily meets the requirements. The problem I'm having is finding the necessary forms and who do they get submitted to. I have the DA 1256, listed in the FRG handbook (USARC Regulation 608-1). But I can find the "U.S. Army Reserve Annual Volunteer Award Nomination Form" which if I am reading the reg right needs to be attached to the DA 1256. I have looked through AKO, Forms, and various Googled information but to no avail.

Where can I find this form? Is there a different route we can go to get her this award or is there a "better" award we can look into?

Thanks for the info. According to one of those links the achievements can be written on a blank piece of paper and then the citation on another. That would make things way easier and makes a lot more sense than a mystery form that seems impossible to find. Of course things in the Army that make sense never seem to work out...
