DA 638's are available quickly through your 'forms' on AKO, it's much easier to access them there than to wait for someone to e-mail it.
Here're the bullets on my DA 638 that were approved:
1.)***** was recognized by the USO *** as the July **** "volunteer of the month". He earned this prestigious title by volunteering an amazing 200 hours within a three month time frame. His story was so remarkable it was featured in the locally published **** magazine this month. ***** has assisted the USO by working as a photographer, serving food at events, assisting on USO tours and general assistance at the *** USO *****. His actions displayed the true meaning of our motto "Selfless-Service"
2.) ***** dedicated many of his non-working weekends to assisting the ****** community's AADD program, volunteering over 30 hours in one week. **** assisted the program in a variety of roles, including answering phones, driving the designated drivers, giving directions, and coordinating requests. His resolve to help others was proven by the fact that he paid for taxi fares to the AADD sites since he does not occupy a privately owned vehicle.
For merritorious achievement while volunteering over 230 hours in community service from **** to ****. Your dedication to helping others reflects great credit upon you, ****, the *** department, and the United States Army.
Hope that helps!
